We have established various services aligned with the NHS’s Building the Right Support agenda.

These services cater to individuals who have resided in hospitals, secure facilities, or Assessment and Treatment Units, often situated outside their boroughs. Many of these individuals have complex, profound, and multiple disabilities, as well as behaviours that present significant challenges.

In collaboration with commissioners and clinicians, we offer customized support packages and housing solutions, both in group settings and individually. Our aim is to assist individuals in transitioning from long-term hospital stays, sometimes spanning many years, to more independent living arrangements closer to their families and communities.

Home Care Services for Everyone

We offer a variety of registered care services tailored to individuals with learning disabilities and additional requirements. These encompass physical and sensory disabilities, long-term conditions, and mental health needs. Furthermore, we are equipped to offer medication support in both our care home facilities and other registered services.
The majority of our customers with learning disabilities benefit from our supported living services. The extent of support provided varies based on individual needs, ranging from round-the-clock assistance to a few hours per day. At times, we offer support within properties where we act as landlords, while in other cases, our services are delivered within buildings owned and managed by external parties. These services are frequently tailored for individuals transitioning from residential care or higher-support accommodations. Their purpose is to assist customers in enhancing their independent living abilities within a safe and nurturing environment. Our moving and visiting support services assist individuals within their own residences. Customers may present with diverse disability levels, encompassing physical and sensory requirements, and reside in various accommodation arrangements.
Our services offer diverse daytime activities and collaborate closely with the local community to encourage participation. These activities encompass various health and wellness, recreational, and educational opportunities. We cater to individuals with various needs, including those who attend with or without caregiver support. Additionally, we offer transportation assistance and/or travel training as needed.
We collaborate closely with commissioners, families, and local authorities to facilitate the transition of customers from secure hospital settings, some of whom may have lived there for over 15 years, back to their homes.
